His back injury is showing no signs of improvement, despite a two-month lay-off.
Manchester United boss Alex Ferguson admitted he can set no date for a return.
He said: "There's no light there. It's difficult to put a time line on it. Anyone who has a back injury or a sore back knows what I'm talking about. It is a difficult thing."
The central defender is currently just doing a bit of gym work and having injections.
Ferguson added: "We gave him an injection just over two weeks ago and had to let that settle for a week.
"He did nothing for a week and then got back into the gym work. We're hoping that the pain goes away."
The player knows he will need to prove to coach Fabio Capello he is fit enough and strong enough to go with England to the World Cup in South Africa in the summer.
Ferguson added: "He's quite a composed lad, Rio, quite laid back. But inwardly he'll be feeling it.
"He's a footballer and footballers like to play. He'll be thinking about all sorts, the run-in to the league and important games coming up. He's missing out on that."
